Please complete this form and email it to Melissa Pabon or print and mail it to PO Box 98, Crestwood, KY 40014. You may also submit your report data online through the ACMR website, here. Please note: You will still need to submit your annual total ACMR data in January, even if you reported data monthly. 

Annual Church Ministries Report

Annual Church Ministries Reports are due by January 31 of each calendar year. To complete your church’s ACMR online, please wait until after January 1, then click here to access the link. If you prefer, you may still complete the hard copy form that is mailed to your church. If so, make sure to:
